प्रार्थी आनॅलाईन दाखिला फार्म भरते समय निम्नलिखित दस्तावेज (जो लागू हो) अपने तैयार रखें।

• अपना निजी मोबाईल नं० एवं ई-मेल आई. डी.
• आधार कार्ड
• परिवार पहचान पत्र ( PPP)
• अपना नवीनतम फोटो
• हस्ताक्षर
• शैक्षणिक योग्यता ( 10वीं/12वीं/ GNM Certificate ) के प्रमाण-पत्र
• चरित्र प्रमाण पत्र ऐसे प्रार्थी (केवल पुरुष) जिन्होंने दाखिले के लिए न्यूनतम शैक्षणिक योग्यता वर्ष 2023 से पहले उत्तीर्ण की हे  वे अंतराल अवधि (Gap period) एवं चरित्र प्रमाण-पत्र नोटरी पब्लिक से सत्यापित करवाकर दाखिले के समय प्रस्तुत करेंगे। महिला प्रार्थी केवल अंतिम शिक्षण संस्थान के मुखिया द्वारा बनाया गया चरित्र प्रमाण – पत्र प्रस्तुत करेगी | ( यदि लागू हो तो )
• जाति प्रमाण-पत्र SC/SC deprived/BCA/BCB/PH/ EWS (of General category) /EX-Serviceman (ESM) etc. ( यदि लागू हो तो )
• हरियाणा स्थाई निवासी प्रमाण-पत्र /अडोमोसाईल
• पिताविहीन / अनाथ / विधवा / विधवा की संतान का प्रमाण-पत्र ( यदि लागू हो तो )

दाखिला प्रक्रिया

• The schedule of counselling, admissions and start of classes shall be in conformity with the schedule laid down by Indian Nursing Council / State Government/Haryana Nurses & Nurse Midwives Council.

Qualification

• ANM Course : 12th Class Passed
• GNM Course : 12th Class Passed with English and must have 40% marks in aggregate & English Subject ./ ANM Certificate
• MPHW Course : 12th Class Passed
